Consult with you GP online using eConsult

eConsult is a platform that Wallingbrook Health Group provides to allow patients to consult with their own NHS GP simply by completing a quick online form. It helps our GPs to deliver better access to our registered NHS patients by providing a round-the-clock portal where patients can enter their symptoms and receive instant self-help advice, together with signposting to NHS 111, pharmacies and other healthcare services.

Practice Questionnaires and Forms

Home Blood Pressure Reading Forms

High blood pressure, or hypertension, rarely has noticeable symptoms. But if untreated, it increases your risk of serious problems such as heart attacks and strokes.

We encourage our patients to check their blood pressure; this can be done at home using a Home Blood Pressure Monitor or at Wallingbrook Surgery using the Blood Pressure Monitoring machine in the MOT Bay situated in the waiting room.

If you take your blood pressure at home please use the Home Blood Pressure Readings Form below to record your readings, hand in the form to our reception team who will then record your average home reading into your records. The home reading average will give us an accurate reading and help us advise if you need treatment.
