New Patients

How to Register

Registration forms can be collected from any of the surgeries.

Please fill these in as soon as possible and return to the surgery, where you will be offered a new patient check with the Health Care Assistant.

New Patient Health Check

Newly registered patients will be invited to attend a health check with one of our Health Care Assistants.

This will include measuring height, weight, blood pressure, a urine test and obtaining details of past medical history, family and health problems, thereby recording important health details before the medical records are received.
