HOW TO REGISTER?
Registration forms can be collected from any of the surgeries. Please fill these in as soon as possible and return to the surgery, where you will be offered a new patient check with the nurse.

MINOR SURGERY
The Doctors perform a wide range of minor surgery procedures in the Health Centre. A Doctor or Receptionist can advise you of the necessary arrangements.  

CONFIDENTIALITY
All aspects of your treatment will be dealt with in the strictest confidence. If you wish to speak to a Receptionist in private, please tell her or pass her a note so indicating.  

ACCESS TO MEDICAL RECORDS
You have a right to see your health records subject to any limitations in the law. If you wish to do so, please discuss the arrangements with the Practice Manager or Doctor.

PATIENTS COMMENTS
We are always keen to improve our services. Comments on the provision of services by the practice are welcomed by letter or in discussion with the Practice Manager, or the Doctor during an appointment, or directly with the staff concerned. Written comment can either be dropped into the mailbox by the front door of the Health Centre or entered in the suggestions/ comments book which can be found in the waiting room at Chulmleigh.

VIOLENT OR ABUSIVE BEHAVIOUR
Wallingbrook Health Centre believes that threat of, or actual, violent and aggressive behaviour towards staff is totally unacceptable. Should any incident occur, appropriate action will be taken.  

PATIENT ADVICE & LIAISON SERVICE
You can access the Patient Advice and Liaison Service by telephoning 01271 335405 between 9am and 4pm. If you wish to meet with a member of the team, they can arrange to meet you. Alternatively you can also email on pals@ndevon.swest.nhs.uk
